Person-Centred Living Philosophy

At Franklin Gardens Long Term Care Home, we recognize and value each resident as an individual, with unique strengths and preferences, and as a citizen with rights and influence.

 

We place equal importance on the emotional, social, spiritual, and cultural well-being of residents, alongside their physical and mental well-being. Our environment is founded on kindness to ensure residents live with their self-defined dignity, autonomy, purpose and security. Our care and services are based on the best prevailing practice models.  

 

We strive to create warm, welcoming home-like environments that are reflective of the residents’ preferences and personalities. 

 

We encourage residents to set personal goals, to stay connected and active within, and outside, the home. As members of a collective community, we welcome external partners into the home to support the well-being of residents.

 

Our approach to care services:

  • is interprofessional and collaborative, engaging residents, families, and team members in decision-making that reflects resident preferences and needs; 

  • aims to maximize and maintain the independence of residents and includes the Restorative Care Philosophy;

  • values continuity of care and the Primary Care model: with meaningful and stable relationships between residents and team members across all roles.
